Can you take this survey?

By Sandra Lee Schubert | April 28, 2010

Do you have a few minutes?

I am working on a book project with a client and gathering some information for it. At the bottom of the page you’ll find a survey. Please take some time to answer the question below. I’ll need to obtain at least fifty responses so feel free to forward the survey with this text intact. Thank you in advance for helping out with this important project.

What is death?

Here are some starter questions. Please free to add anything else around this question.

How do you perceive death? Do believe in an afterlife or reincarnation? Is there a heaven or hell? Do you think it is final act? You die and that is it? Have you thought about your mortality? Have you had a near death experience? What have been your experiences with loved ones or friends and death?

Please add any other experiences or thoughts about this topic.

Thank you for taking the time and thought in answering this survey. We appreciate the effort in assisting us in this worthwhile project.

We are tracking the survey results for future projects when you get to number six please fill where you first saw the survey.

Sample answers for question six: 6. Please let us know where you saw this survey: Amplify-Sandra, Facebook-Sandra, Twitter-Sandra, Blog-Sandra, Email-Sandra
